The Troop Treasurer's Report must be completed every year. |
| Driver Safety Checklist |
This form is used to c ollect information for any person who will drive girls during Girl Scout activities. |
| General Patch Order Form |
Order patches designated as "no cost" in the Council's Patch Program book. |
Health History & Health
Exam Records |
Leaders may use this form to record their girls' health history; also required for parents to fill out for summer camp registration. |
| Incident Report |
This report must be completed and returned to the Council in the event of incident or injury at a Girl Scout meeting or activity. |
| Lapsed Troop Report |
This form must be completed if a troop is no longer meeting. |
| Money-Earning Project Application |
Complete this form prior to any troop or group activity where money is raised. |
| Parent/Guardian Permission Form |
Parents must complete this form to give permission for their daughter to participate in Girl Scout activities or programs. |

Use this form to register for Council-sponsored program opportunities. See Program Planner for details. |

Permission Form |
Use this form when special permission is required by parents for certain activities. (See Safety-Wise.) |

This form is to be used by Troop Leaders and must accompany Program Registration Form. |

Complete this form every year or when a troop opens a bank account. |

Bronze Award Packet &
Final Report |
Summarizes activities for Bronze Award and includes final report form to be completed. |
| Girl Award Nomination Form |
Several awards recognizing registered Girl Scouts (see Program Planner page 51 for details). Deadline: February 8, 2008. |
| Gold Award Project Final Report |
The Gold Award Project is not considered complete until a final report and log of hours is submitted and approved. |

This sheet provides a summary of prerequisites, the application process, and requirements for completing the Gold Award Project. |
| Gold Award Project Proposal |
This form must be completed and submitted to the Gold Award Review Committee by the first Monday of the month prior to beginning the project. |

This award is issued by the President's Council on Service and Civic Participation on behalf of the President of the United States. |

This award recognizes outstanding service to two or more service areas. |
Leadership Development Pin
Application |
This recognition is for registered adult Girl Scouts who have completed one year as leader, assistant leader, or group coordinator, plus training and other requirements. |
| Outstanding Leader |
This award recognizes a registered Girl Scout adult in a troop leader or assistant troop leader capacity. |
| Outstanding Volunteer |
This award is for outstanding service of a volunteer (other than a leader). |
| President's Award |
This award recognizes efforts of a service area in moving toward the Council's goals and objectives. |
| Thanks Badge |
This award recognizes outstanding service that benefits the Council or entire Girl Scout organization. |
| Thanks Badge II |
This award recognizes continued outstanding service by a previous Thanks Badge recipient. |
